Passports for Minors Under 16 (minors under 16 can NOT use the renewal form) Quick Facts Who Needs One: Every minor regardless of age traveling overseas must have a valid passport Valid For: 5 years Submission Method: Passport applications for minors under age 16 must be submitted in person at a passport agency or authorized passport application acceptance facility. The minor must be present. Both parents or guardians should appear with the minor. If both parents/guardians cannot be present, see parental consent for additional document requirements. What you need to apply for a minor's passport Evidence of U.S. Citizenship Photo Identification Parental Consent Passport Photo Application Forms Passport Fees 1. Evidence of U.S. Citizenship You must submit one of the following original certified documents as primary evidence of U.S. citizenship for your child (photocopies are not acceptable): Certified U.S. Birth Certificate (must meet all of the following requirements): Issued by City, County, or State of birth Lists bearer's full name, date of birth, and place of birth Lists parent(s) full names Has date filed with registrar's office (must be within one year of birth) Has registrar's signature Has embossed, impressed, or multicolored seal of registrar Previous U.S. Passport (may be expired, must be undamaged) Consular Report of Birth Abroad Certificate of Naturalization Certificate of Citizenship If your child was born in the United States and cannot obtain a certified birth certificate that meets all the requirements listed above, you will need to submit a combination of documents as secondary evidence of U.S. citizenship, such as: Delayed birth certificate (filed after one year of birth) Hospital records Religious records (certificate of baptism, communion, blessing, family bible, etc.) Early school records If your child has never had a birth certificate filed, see required documents. If your child has previously been issued a passport or Consular Report of Birth Abroad, and it is not available to submit with your application, you may request a file search . If your child was born outside the United States and has U.S. citizenship by birth to one or more U.S. citizen parents, or derived U.S. citizenship through the naturalization of parents, and does not have a Certificate of Citizenship, see required documents .

Gender Transition Applicants We can issue two different kinds of passports to you depending on where you are in your gender transition. You must provide extra documentation in both cases. Status of Gender Transition Kind of Passport Validity of Passport Completed Full Validity 10 years In Process Limited Validity 2 years Requirements Whether your gender transition is "in process" or "completed," you must apply using Form DS-11 . Please include the following, in addition to the regularly-required documents: ID that resembles your current appearance Passport photo that resembles your current appearance Proof of legal name change (if applicable) A physician statement that indicates you have either completed or are in process of treatment for gender transition Please see Apply in Person for all regularly-required passport documents. Replacing a Limited-Validity Passport? If your passport book was limited to 2 years because your gender transition was "in process" (as indicated in the chart above) and you have now completed transition, you can apply for a replacement full ten-year validity book at no additional cost by submitting Form DS-5504 . Please also submit the following: Your limited passport book A physician statement indicating you have completed appropriate clinical treatment for gender transition Physician's Statement The signed original statement from the attending medical physician must be on office letterhead and include: Physician’s full name Medical license or certificate number Issuing state or other jurisdiction of medical license/certificate Address and telephone number of the physician Language stating that he or she is your attending physician and that he or she has a doctor/patient relationship with you Language stating you have completed or are in process of appropriate clinical treatment for gender transition to the new gender (male or female) Language stating “I declare under penalty of perjury under the laws of the United States that the forgoing is true and correct.” Description of specific treatments is not required. However, the certification from a licensed physician, who has treated the applicant or reviewed and evaluated the medical history of the applicant regarding the change in gender, shall be based on the physician's judgment regarding the applicant's treatment needs, and made in accordance with standards and recommendations of the World Professional Association for Transgender Health (WPATH ), recognized as the authority in this field by the American Medical Association (AMA ). Passports for Minors Age 16 and 17 Valid For: 10 years Submission Method: MUST apply in person on Form DS-11 Parent Identification: A parent can identify a minor that does not have his or her own identification Parental Consent Minors ages 16-17 with their own identification can apply for a passport by themselves; however, it is recommended that at least one parent appear in person with the minor to identify him/her and to show parental awareness. Parental Awareness Having a parent accompany the minor when applying, Listing a parent as the Emergency Contact on the application, Using a parent’s check to pay the passport fees, or A signed statement consenting to issuance of a passport from at least one parent (should be accompanied by a photocopy of that parent’s ID). Board Of Equalization Note: Hearings will be held at the 'Annex' 155 Towne Lake Pkwy Woodstock GA 30188 AM Only: 1-5-26 Hearing Officer: 1-8-26 Hearing Officer: 1-9-26 Appeal Reschedule Form Appeal Withdrawal Form Appointments to the Board of Equalization Ad Application for Board of Equalization CHEROKEE COUNTY BOARD OF EQUALIZATION To the citizens of Cherokee County: During the last legislative session, Senate Bill 346 was passed and signed into law by Governor Sonny Perdue on May 13, 2010, with an effective date of January 1, 2011. This new law sets forth the changes to the Board of Assessors and the Board of Equalization. One important section of the bill changes the oversight responsibilities from the Board of Equalization; and assigns these duties to the Clerk of Superior Court. This was done to ensure a neutral third party, the Clerk, would administer the operations in an effort to alleviate the appearance of impropriety and partiality. As the Clerk of Superior Court, I take these responsibilities very seriously. It is important to understand that as the manager of these duties, I am responsible for the process NOT the decision rendered by the Board of Equalization. My office will always stand in the middle as the keeper of the record and will continue to be the unbiased holder of all filed paperwork. We have nothing to do with property values or decisions made by the board. I will make certain the process is fair to everyone and that each person is heard and treated with respect. The members of the Board of Equalization are appointed by the Grand Jury; they are not employees of Cherokee County. The board members are citizens of Cherokee County just like you and me. Their appointment is not by a taxing department nor is their oversight by a taxing authority. Each board member takes an oath of office to faithfully and impartially discharge his or her duties as identified in the Georgia Code. Members are required to have forty (40) hours of training when first appointed and eight (8) hours of continuing education each subsequent year. As always, your comments are welcomed and encouraged.
